ABSTRACT

Secondary bacterial peritonitis [SBP] is one of the common surgical emergencies in the world [1,2]. Purpose of the present study was to establish the common cause of SBP in our environment. Prospectively; we studied ninety patients [age: 3 to 63 years] admitted to El-Thoura Teaching Hospital; Al-Beida- Libya, in the period from January to December 2005. Perforated appendicitis was found in 31 [34.4%], followed by; perforated peptic ulcer 25 [27%], acute pancreatitis 9 [10%], acute cholecystitis with peritonitis 8 [8.8%],small bowel perforations 8 [8.8%] [3 cases trauma, 2 ischemia, 2 Crohns disease and one case typhoid perforation], large bowel perforations 6 [6.6%] [3 cases trauma, 2 diverticulitis perforation, one case tumor]. We found that perforated acute appendicitis followed by peptic ulcer perforation is the most common cause of peritonitis in our environment. On other hand; this study abdominal trauma is the main cause of bowel perforation in our environment

A síndrome compartimental abdominal (SCA) é um conjunto de alterações fisiopatológicas, principalmente sobre os sistema cardiovascular, respiratório e renal, decorrentes da elevação aguda da pressão intra-abdominal (PIA). Possui como importantes fatores desencadeantes o trauma abdominal, peritonite, pancreatite, transplante hepático, ascite volumosa, laparotomia abreviada e a cirurgia videolaparoscópica. Os autores revisam aspectos atuais acerca da fisiopatologia, etiopatogenia e terapia desta afecção


The abdominal compartment syndrome (ACS) is a group of systemic alterations, mainly on the cardiovascular and respiratory systems, current of the sharp elevation of the intra-abdominal pressure (IAP). Important risk factors are the abdominal trauma, peritonitis, pancreatitis, hepatic transplant, voluminous ascites, abbreviated laparotomy and the videolaparoscopic surgery. Objective: the aim of this paper is to review the main aspects of this affection

ABSTRACT

Os autores estudaram os aspectos fisiológicos e técnicos da execução da laparotomia abreviada (LA), um método utilizado na cirurgia do trauma que consiste basicamente em empregar rápidas medidas de controle de jemorragia, evitar ou limitar extravasamento de conteúdo gastrintestinal e realizar o rápido fechamento do abdome, em pacientes traumatizados que desenvolvam hipotermia, acidose e coagulopatia. As principais indicações para o seu emprego são: incapacidade de realizar hemostasia direta; necessidade de rapidamente encerrar o procedimento em paciente coagulopático e hipotérmico; fechamento do abdome para proteção das vísceras e minimização das perdas de proteínas e calor. A utilização destes métodos tem demonstrado ser eficaz e capaz de salvar pacientes vítimas de trauma
